1. Counselor Trainee (CT) Information:
a. Easy Steps to Counselor Trainee
b. CT Frequently Asked Questions - For Counselor Trainees (CT) to find answers to most any question they may encounter about holding a CT.
c. CT Initial Application - For Counselor Trainee (CT) to register their clinical supervisor (CT fills in their part & provides to supervisor to complete and submit).
d. CT Extension - For Counselor Trainee (CT) to update or change information affecting their existing registration including change of supervisor.
e. Internship Supervision Evaluation Form - For counseling interns to obtain supervisor evaluation of intern (one form required for each supervisor and/or location w/i 30 days of completion).

3. Professional Counselor - Clinical Resident (CR) Information:
a. CR Application Instructions
b. CR Supervision Requirements for PCC- For Clinical Resident (CR), lists supervision requirements for earning hours under supervision.
c. CR Application - Online - For Clinical Resident (CR) to apply for registration and document their initial clinical supervisor (CR complete application & supervisor is emailed to complete process). You must have an active PC license to use a counselor resident registration.
d. CR Application - Paper - For Clinical Resident (CR) to apply for registration and document their initial clinical supervisor (CR complete application & supervisor is emailed to complete process). You must have an active PC license to use a counselor resident registration.
e. Form to document supervised hours - For evaluation of PC working under PCC supervision as a clinical resident to document hours. You must have an active PC license to use a counselor resident registration. For Clinical Resident to obtain supervisor evaluation (at completion of 1,500 and 3,000 hours of supervision). You must have two full years of supervision documented.
