INSTRUCTIONS AND INFORMATION FOR COMPLETING THE SOCIAL WORKER ASSISTANT REGISTRATION APPLICATION

The appropriate fee must accompany the application. Payment must be made either by certified check or money order, made payable to "CSWMFT Board".


1. Enter your name as you would like it printed on your license. Be sure to include your social security number.

2. Do not forget to include your postal zip code.

3. Please provide available telephone numbers, including the area codes.

4. Please provide a valid email as we try to eliminate mail as much as possible.

5. The Memo of Understanding must be completed by all applicants.

6. Notary - your application must be notarized.

7. Applicants are required to have an official college transcript, showing conferred degree(s), sent directly to the Counselor, Social Worker & Marriage and Family Therapist Board. Student copies are not acceptable.


ISSUANCE OF THE LICENSE IS COMPLETED AT THE NEXT BOARD MEETING AFTER RECEIPT OF APPLICATIONS AND FEES

Board meetings are the third Thursday of January, March, May, July, September and November.

Applications and transcripts have to be received in the office at least ten days prior to the board meeting to ensure processing by the staff for the board meeting. Any applications received less than ten days prior to the board meeting may have to wait until the following board meeting for approval.

Failure to complete all areas as instructed will delay the processing of your licensure application.
